      目的 研究中国社区老年男性细胞外水分比率(ECW/TBW)与身体机能指标的相关性,并探讨其是否可以作为评价老年男性身体机能的客观指标。方法 采用横断面设计,共纳入588名≥65岁的社区老年男性。采用生物电阻抗分析法评估ECW/TBW;采用Fried衰弱表型诊断是否为衰弱前期,测试并记录握力、步速及5次起坐时间。分析衰弱前期患者的临床特征,并采用Spearman进行ECW/TBW与躯体功能指标的相关性分析,应用多元线性回归分析确定与躯体功能指标独立相关的因素,应用多元Logistic回归分析衰弱前期的独立影响因素。结果 单因素分析显示,衰弱前期组ECW/TBW高于非衰弱组(P<0.001)。相关分析显示,社区老年男性较高的ECW/TBW与握力和步速降低以及5次起坐时间延长有关;在调整了混杂因素后,ECW/TBW与握力、步速和5次起坐时间显著且独立相关。多元Logistic回归分析显示,ECW/TBW(OR=0.431,95%CI:0.200~0.927,P=0.031)、5次起坐时间(OR=0.936,95%CI:0.891~0.983,P=0.008)和PBF(OR=1.425,95%CI:1.602~1.912,P=0.018)是老年男性衰弱前期的独立影响因素。结论 ECW/TBW反映了社区老年男性的身体功能状况,可能是与衰弱早期发展相关的重要指标。

      Objective To investigate the correlation between extracellular-water/total body water ratio(ECW/TBW)and physical function indicators in community-dwelling elderly Chinese men,and explore whether ECW/TBW can serve as an objective marker for evaluating physical function in this population.Methods A cross-sectional study design was employed.A total of 588 community-dwelling men aged≥65 years were enrolled.Bioelectrical impedance analysis was used to assess ECW/TBW.The Fried frailty phenotype was applied to diagnose pre-frailty.The grip strength,gait speed and 5-repetition sit-to-stand time were measured and recorded.Clinical characteristics of patients with pre-frailty were analyzed.Spearman correlation analysis was used to examine the relationship between ECW/TBW and physical function indicators.Multiple linear regression was performed to identify factors independently associated with physical function indicators,and multiple Logistic regression was used to analyze independent influencing factors for pre-frailty.Results Univariate analysis showed that the pre-frailty group had significantly higher ECW/TBW than the non-frail group(P<0.001).Correlation analysis showed that a higher ECW/TBW in community-dwelling elderly men was associated with reduced grip strength and gait speed and prolonged 5-repetition sit-to-stand time.After adjusting for confounding factors,ECW/TBW remained significantly and independently correlated with grip strength,gait speed,and 5-repetition sit-to-stand time.Multiple Logistic regression analysis indicated that ECW/TBW(OR=0.431,95%CI:0.200-0.927,P=0.031),5-repetition sit-to-stand time(OR=0.936,95%CI:0.891-0.983,P=0.008),and PBF(OR=1.425,95%CI:1.602-1.912,P=0.018)were independent influencing factors for pre-frailty in elderly men.Conclusion The ECW/TBW reflects the physical function status of community-dwelling elderly men and may be an important indicator associated with the early development of frailty.
