      目的 采用低负荷血流限制训练为衰弱老人进行为期12周的治疗,观察其对衰弱老人体适能、下肢肌力、人体成分的影响。方法 纳入64例2022年1月至2024年12月期间入住上海市保健医疗中心老年科的老年衰弱患者,按照随机数字表法,分为观察组32例和对照组32例。对照组进行低负荷抗阻训练,观察组进行低负荷血流限制训练。在训练4周、12周后,比较2组体适能、下肢肌力、人体成分变化。结果 训练4周后,2组6 min步行距离,30 s坐站试验,下肢最大等长肌力(股四头肌、腘绳肌、腓肠肌)各指标均升高,8英尺起立行走测试下降,骨骼肌质量、体脂百分比无变化。12周后,2组骨骼肌质量上升、体脂百分比下降,6 min步行距离、30 s坐站试验、下肢肌力进一步上升,8英尺起立行走测试进一步下降,且观察组变化更明显,差异均有统计学意义。(P<0.05)。结论 12周的低负荷血流限制训练对衰弱老年患者的体适能、下肢肌力和人体成分的改善效果显著优于低负荷抗阻训练,其疗效优势可能需要长期干预(> 4周)才能充分显现。

      Objective To treat frail elderly patients by a 12-week low-load blood flow restriction training(LL-BFRT),and observe its effects on the physical fitness,lower limb muscle strength and body composition.Methods A total of 64 frail elderly patients admitted to Department of Rehabilitation Medicine,Shanghai Health and Medical Center between January 2022 and December 2024 were enrolled.They were randomly divided into observation group(n=32)and control group(n=32)using a random number table method.The control group performed low-load resistance training(LLRT),while the observation group performed LL-BFRT.After 4 weeks and 12 weeks of training,the changes in physical fitness,lower limb muscle strength and body composition were compared between the two group.Results After 4 weeks of training,both groups showed improvements in 6-minute walk distance(6MWD),30-second sit-to-stand test(30sSTST),and maximal isometric strength of the lower limb muscles(quadriceps,hamstrings,gastrocnemius),while the 8-foot up-and-go test(8-FUGT)time decreased.There were no significant changes in skeletal muscle mass or body fat percentage.After 12 weeks of training,both groups showed increased skeletal muscle mass,decreased body fat percentage,further improvements in 6MWD,30sSTST,and lower limb muscle strength,and a further reduction in 8-FUGT time.The changes in the observation group were more significant,and all differences were statistically significant(P<0.05).Conclusion The improvement effects of the 12-week LL-BFRT on the physical fitness,lower limb muscle strength and body composition in frail elderly patients is significantly better than that of the LLRT.Its therapeutic advantages may require long-term intervention(>4 weeks)to be fully manifested.
