| 目的 探讨18氟-氟代脱氧葡萄糖正电子发射断层显像/计算机断层扫描(18F-FDG PET-CT )对不同病理类型卵巢肿瘤的诊断价值。方法 选取2020年1月至2024年12月期间行18F-PET-CT检查并行手术切除的卵巢肿瘤患者,统计分析不同病理类型卵巢肿瘤的18F-FDG PET-CT影像表现。结果 共纳入患者80例,年龄39~78岁,平均年龄(71.2±7.5)岁。其中上皮源性肿瘤45例(恶性35例,中间型5例,良性5例),35例上皮源性恶性肿瘤表现为形态不规则、边界不清的软组织肿块,标准化摄取值(SUV) max范围为4.5~12.0; 5例中间型上皮源性肿瘤边界相对清晰,密度欠均匀,其中2例诊断为恶性,SUVmax范围为3.5~5.5,3例诊断为良性,SUVmax均<2.5; 5例良性肿瘤边界清晰,密度均匀,SUVmax<2.5。生殖细胞肿瘤20例,12例成熟畸胎瘤,SUVmax范围为1.5~3.0,5例未成熟畸胎瘤,SUVmax范围为5.0~10.0,1例无性细胞瘤,SUVmax为11.0,2例卵黄囊瘤,SUVmax分别为3.8、7.3。性索间质肿瘤10例,良性4例为卵泡膜纤维瘤,SUVmax均<2.5,6例颗粒细胞瘤,SUVmax范围为3.0~7.0。转移性肿瘤5例,SUVmax范围为7.0~13.0。上皮源性肿瘤总诊断符合率为82.2%(37/45),生殖细胞肿瘤总诊断符合率85.0%(17/20),性索间质肿瘤总诊断符合率为8/10,转移瘤诊断符合率为5/5。结论18F-FDG PET-CT对卵巢肿瘤具有一定的诊断价值,通过其影像表现和代谢特点可辅助鉴别诊断不同类型的卵巢肿瘤。 |

| Objective To explore the diagnostic value of 18 F-fluorodeoxyglucose positron emission tomography/computed tomography(18 F-FDG PET-CT)in different pathological types of ovarian tumors.Methods Patients with ovarian tumors who underwent 18F-FDG PET-CT examination and surgical resection between January 2020 and December 2024 were selected.The imaging manifestations of different pathological types of ovarian tumors were statistically analyzed.Results A total of 80 patients were included,aged 39-78 years,with a mean age of(71.2±7.5)years.Among them,45 cases were epithelial origin tumors(35 malignant,5 intermediate,5 benign).The 35 malignant epithelial tumors presented as soft tissue masses with irregular shapes and unclear boundaries,and the maximal standardized uptake value(SUVmax)range was 4.5-12.0.The 5 intermediate-type epithelial tumors had relatively clear boundaries and heterogeneous density.Among them,2 cases were diagnosed as malignant(SUVmax 3.5-5.5),while the other 3 ones were diagnosed as benign(SUVmax<2.5).The 5 benign tumors showed clear boundaries,homogeneous density,and SUVmax<2.5.There were 20 cases of germ cell tumors,including 12 mature teratomas(SUVmax 1.5-3.0),5 immature teratomas(SUVmax 5.0-10.0),1 dysgerminoma(SUVmax 11.0)and 2 yolk sac tumors(SUVmax 3.8,7.3),respectively.There were 10 sex cord-stromal tumors,including 4 benign(follicular membrane fibromas,SUVmax<2.5)and 6 granulosa cell tumors(SUVmax 3.0-7.0).There were 5 metastatic tumors(SUVmax 7.0-13.0).The overall diagnostic concordance rate for epithelial tumors was 82.2%(37/45),for germ cell tumors85.0%(17/20),and for sex cord-stromal tumors 8/10.The diagnostic concordance rate for metastatic tumors was5/5.Conclusion 18 F-FDGPET-CT has certain diagnostic value for ovarian tumors.Its imaging manifestations and metabolic characteristics can assist in the differential diagnosis of different types of ovarian tumors. |
