| 目的 观察老年患者术前2h口服碳水化合物溶液的胃排空时间,探讨老年患者术前2h口服碳水化合物溶液的安全性.方法 2020 年10 月至2021 年3 月,纳入复旦大学附属华东医院手术当天第 1 台择期手术患者 66 例.根据年龄分为老年组(n=33)和中青年组(n=33).术前2h受试者右侧卧位测量空腹胃窦部横截面积(CSA).口服 5%葡萄糖溶液400 mL后,分别于饮用后5、30、60、75、90、105 min测量胃窦部CSA.记录患者胃排空时间.结果 66 例受试者年龄22～90 岁,平均(60.4±16.4)岁,其中男性47 例、女性19 例.与中青年组相比,老年组患者胃排空时间较长,差异有统计学意义(60.00 min比75.00 min,P＜0.05).所有患者均在105 min内完成胃排空.2 组各测量时间点胃窦部CSA差异均无统计学意义(F=0.702,P=0.408).胃排空时间与年龄、身高、体重、体质量指数、空腹胃窦部CSA和禁饮时长均不相关.结论 老年患者术前2h口服5%葡萄糖溶液400 mL可能安全可行. |

| Objective To observe the gastric emptying time of elderly patients taking oral carbohydrate solution 2 hours before surgery and explore the safety of this practice.Methods From October 2020 to March 2021,66 patients scheduled for the first elective surgery on the day of operation in Huadong Hospital Affiliated to Fudan University were enrolled.They were divided into elderly group(n=33)and young/middle-aged group(n=33)based on age.Two hours before the operation,the fasting cross-sectional area(CSA)of the gastric antrum of the patients was measured in the right decubitus position.After oral administration of 400 mL of 5%glucose solution,the antral CSA was measured at 5,30,60,75,90 and 105 minutes after consumption,respectively.The gastric emptying time of patients was recorded.Results The age of 66 subjects ranged from 22 to 90 years,with an average of(60.4±16.4)years,including 47 males and 19 females.The elderly group demonstrated significantly longer gastric emptying time than the young/middle-aged group(75.00 min vs 60.00 min,P＜0.05).All patients achieved complete gastric emptying within 105 minutes.There was significant difference in antral CSA between the two groups at any measured time point(F=0.702,P=0.408).The gastric emptying time was not correlated with age,height,body weight,body mass index,fasting antral CSA,and the duration of preoperative fasting.Conclusion Oral administration of 400 mL of 5%glucose solution 2 hours before surgery for elderly patients may be safe and feasible. |
