      目的 探讨针药结合联合抗阻训练对老年女性骨质疏松症骨密度和骨代谢的影响.方法 选取2023 年5 月至2025 年1 月启东市人民医院收治并确诊骨质疏松症的 120 例老年女性患者,根据其接受的治疗方案分为针药组(n=40)、抗阻训练组(n=40)和联合组(n=40).所有患者行常规基础治疗,在此基础上针药组给予针灸治疗,抗阻训练组给予抗阻运动训练,联合组给予针灸联合抗阻训练治疗,3 组均治疗6 个月.比较 3 组治疗前、治疗 6 个月后的疗效、骨密度(腰椎L1~L4、股骨颈和髋部)、骨代谢指标[骨钙素(BGP)、Ⅰ型原胶原N端前肽(PINP)、Ⅰ型胶原交联C端肽(β-CTX)和碱性磷酸酶(ALP)]、Oswestry功能障碍指数(ODI)、视觉模拟评分(VAS)和中国人骨质疏松症生存质量简明量表(COQOL)评分.记录3 组治疗期间的不良反应.结果 治疗 6 个月后,3 组总有效率差异有统计学意义(χ2=14.132,P=0.007),联合组总有效率优于针药组和抗阻训练组(χ2=7.458,P=0.024).治疗6 个月后,3 组骨密度(腰椎L1~L4、股骨颈和髋部)、骨代谢指标(BGP、PINP、β-CTX和ALP)、ODI、VAS和COQOL评分差异有统计学意义(P<0.05);联合组治疗前后的骨密度(腰椎L1~L4、股骨颈和髋部)、骨代谢指标(BGP、PINP、β-CTX和ALP)、ODI、VAS和COQOL评分的差值大于针药组和抗阻训练组(P<0.05).研究期间3 组不良反应发生率差异无统计学意义(P>0.05).结论 针药结合联合抗阻训练相较于单一治疗能一定程度改善老年女性骨质疏松症患者的骨密度与骨代谢,且安全性较高.

      Objective To investigate the effects of acupuncture combined with medication and resistance training on bone mineral density and bone metabolism in elderly women with osteoporosis.Methods A total of 120 elderly female patients with osteoporosis who were admitted to and diagnosed from May 2023 to January 2025 were selected.According to the treatment regimens they received,they were divided into acupuncture group(n=40),resistance training group(n=40),and combined group(n=40)by envelope method.All patients received routine basic treatment.On this basis,acupuncture group was given acupuncture treatment,resistance training group was given resistance exercise training,and combined group was given acupuncture combined with resistance training,and all 3 groups were treated for 6 months.Efficacy,bone mineral density(L1-L4,femoral neck and hip),bone metabolism indicators[bone Gla-protein(BGP),procollagen Ⅰ N-terminal propeptide(P1NP),beta-isomerized C-terminal telopeptide of type Ⅰ collagen(β-CTX),alkaline phosphatase(ALP)],Oswestry disability index(ODI)scores,visual analogue score(VAS)scores,and Chinese osteoporosis-targeted quality-of-life(COQOL)scale scores were collected and analyzed in three groups.Adverse reactions during the treatment period were compared among 3 groups before treatment and after 6 months of treatment.Results After 6 months of treatment,the difference in total effective rate among 3 groups was statistically significant(χ2=14.132,P=0.007),and total effective rate of combined group was better than that of acupuncture and resistance training groups(χ2=7.458,P=0.024).After 6 months of treatment,the differences in bone mineral density(L1-L4,femoral neck and hip),bone metabolism indexes(BGP,PINP,β-CTX and ALP),ODI,VAS and COQOL scores among the three groups were statistically significant(P<0.05);the difference in bone mineral density(L1-L4,femoral neck and hip),bone metabolism indexes(BGP,PINP,β-CTX and ALP),ODI,VAS and COQOL scores before and after treatment was greater in combined group than in acupuncture group and resistance training group(P<0.05).There was no statistically significant difference in the incidence of adverse reactions among the 3 groups during study period(P>0.05).Conclusion Acupuncture combined with combined resistance training improves bone mineral density and bone metabolism to a certain extent compared with monotherapy in elderly women with osteoporosis,and boasts high safety.
