      目的 分析初发超重、肥胖2 型糖尿病(T2DM)老年患者持续皮下泵入贝那鲁肽后胰岛功能的变化,为肥胖、初发的 T2DM 老年患者的诊治提供临床依据.方法 选取2022 年1 月至2023 年8 月在医院内分泌科门诊或住院诊治的40 例初发超重、肥胖 T2DM 老年患者,随机分为贝那鲁肽组及对照组,每组20 例.收集2 组患者的一般情况、血糖、胰岛功能等临床指标,分析贝那鲁肽强化降糖治疗对患者胰岛功能的影响.结果 2 组治疗前体重、体重指数(BMI)、腰围水平,治疗前后腰围、糖代谢指标值差异均无统计学意义(P>0.05).治疗后,2 组体重、BMI、腰围水平均小于同组治疗前,且贝那鲁肽组体重及 BMI 治疗前后差值大于对照组;糖化血红蛋白及空腹血糖水平均低于同组治疗前,对照组稳态模型评估胰岛素抵抗指数水平低于同组治疗前;差异均具有统计学意义(P<0.05).结论 短期贝那鲁肽强化治疗可显著降低 T2DM 患者的体重、BMI、血糖水平,对胰岛功能无明显影响.

      Objective To analyze the changes in pancreatic islet function in overweight or obese elderly patients with newly diagnosed type 2 diabetes mellitus(T2DM)after continuous subcutaneous infusion of beinaglutide,and provide clinical evidence for the diagnosis and treatment of obese elderly patients with newly diagnosed T2DM.Methods A total of 40 overweight or obese elderly patients with newly diagnosed T2DM treated in Outpatient or Inpatient Department of Endocrinology from January 2022 to August 2023 were selected and randomly divided into beinaglutide group and control group,with 20 patients in each group.Their general information,blood glucose,pancreatic islet function,and other clinical indicators were collected.The effect of intensive hypoglycemic treatment with beinaglutide on the pancreatic islet function was analyzed.Results There were no statistically significant differences in body weight,body mass index(BMI),waist circumference before treatment,as well as the differences in waist circumference and glucose metabolism indicators before and after treatment between the two groups(P>0.05).After treatment,weight,BMI and waist circumference in both groups were lower than those in the same group before treatment,and the differences in weight and BMI before and after treatment in the beinaglutide group were greater than those in the control group.The levels of glycosylated hemoglobin and fasting blood glucose in both groups were lower than those in the same group before treatment,and the homeostasis model assessment of insulin resistance level in the control group were lower than that in the same group before treatment.All differences were statistically significant(P<0.05).Conclusion Short-term intensive treatment with beinaglutide can significantly reduce the body weight,BMI and blood glucose of T2DM patients,and has no significant effect on pancreatic islet function.
