      目的 基于中国健康与养老追踪调查2020 年数据,探讨不同慢性病数量下老年人内在能力(IC)水平及其5 个维度的差异,并在控制人口学特征、社会经济状况及健康行为等因素后,分析慢性病数量与 IC 之间的关联.方法 本研究共纳入6 516 名老年人,并对其人口学特征进行描述分析.采用单因素方差分析比较不同慢性病数量组之间 IC 得分的差异;进一步采用多因素有序 Logistic 回归模型,分析慢性病数量与 IC 总分之间的关系.最后,分别以认知、运动、活力、心理及感官5 个功能维度为因变量,构建多因素 Logistic 回归模型,探讨慢性病数量与各功能维度之间的关联.结果 不同慢性病数量的老年人在 IC 得分上存在统计学差异(F=110.126,P<0.001).多因素有序 Logistic 回归结果显示,多重慢性病是影响 IC 得分的独立危险因素(OR=0.68,95%CI:0.58~0.80,P<0.001).多因素二元 Logistic 回归分析表明,慢性病数量主要与运动维度和心理维度相关.与无慢性病者相比,患1 种慢性病者与多重慢性病者在运动维度上维持健康状态的可能性分别下降43%(OR=0.57,95%CI:0.40~0.81)与62%(OR=0.38,95%CI:0.28~0.52);患多重慢性病者在心理维度上保持心理健康状态的可能性下降 35%(OR=0.65,95%CI:0.53~0.79).结论 慢性病累积与老年人IC 呈显著负相关,尤其在与运动维度及心理维度的关联上更为突出.加强多重慢性病人群的综合慢性病管理及功能维护,有助于延缓老年功能衰退,促进健康老龄化.

      Objective To explore the differences in the levels of intrinsic capacity(IC)and its 5 dimensions among the elderly with different numbers of chronic diseases based on data from the China Health and Retirement Longitudinal Study 2020,and analyze the association between the number of chronic diseases and IC after controlling for factors such as demographic characteristics,socioeconomic status,and health behaviors.Methods A total of 6 516 elderly individuals were included in this study,and their demographic characteristics were described.One-way analysis of variance was used to compare the differences in IC scores among different groups with different numbers of chronic diseases.A multivariate ordinal Logistic regression model was further employed to analyze the relationship between the number of chronic diseases and the total IC score.Finally,the 5 functional dimensions(cognition,locomotion,vitality,psychological capacity,and sensory capacity)were taken as the dependent variables,and a multivariable Logistic regression model was constructed.The association between the number of chronic diseases and each functional dimension was explored.Results There were statistically significant differences in IC scores among the elderly individuals with different numbers of chronic diseases(F=110.126,P<0.001).Multivariate ordinal Logistic regression results showed that multimorbidity was an independent risk factor for IC score(OR=0.68,95%CI:0.58-0.80,P<0.001).Multivariate binary Logistic regression analysis indicated that the number of chronic diseases was primarily associated with the locomotion and psychological dimensions.Compared with those without chronic diseases,the individuals with a single chronic disease and those with multimorbidity had a 43%(OR=0.57,95%CI:0.40-0.81)and 62%(OR=0.38,95%CI:0.28-0.52)lower likelihood of maintaining a healthy state in the locomotion dimension,respectively.The elderly individuals with multiple chronic diseases had 35%(OR=0.65,95%CI:0.53-0.79)lower likelihood of maintaining psychological health.Conclusion The cumulative burden of chronic diseases is significantly negatively associated with IC in elderly individuals.The particularly pronounced associations were observed in the locomotion and psychological dimensions.Strengthening comprehensive chronic disease management and functional maintenance in elderly individuals with multimorbidity may help delay functional decline and promote healthy aging.
