      目的 探讨去肾交感神经(RDN)术后序贯不同降压药物对血管紧张素Ⅱ(AngⅡ)诱导高血压老年小鼠血压、肾素--血管紧张素--醛固酮系统(RAAS)和心肌纤维化的作用和影响.方法 采用 AngⅡ将 60 只 18 月龄雄性 C57BL/6小鼠诱导成高血压模型.随机分为 Sham 组序贯生理盐水(Sham-control)、Sham 组序贯沙库巴曲缬沙坦(Sham-ARNI)、Sham 组序贯氨氯地平(Sham-CCB)、Sham 组序贯比索洛尔(Sham-β-blocker)、Sham 组序贯氢氯噻嗪(Sham-DIU)、RDN组序贯生理盐水(RDN-control)、RDN 组序贯沙库巴曲缬沙坦(RDN-ARNI)、RDN 组序贯氨氯地平(RDN-CCB)、RDN 组序贯比索洛尔(RDN-β-blocker)、RDN 组序贯氢氯噻嗪(RDN-DIU)10 组,每组各 6 只.每周监测血压,干预 4 周后取材,收集血浆、肾组织和心肌组织.结果 与Sham-control 组相比,RDN-control 组老年小鼠收缩压(P=0.003)和舒张压(P=0.009)降低.与单纯 RDN 相比,RDN 术后序贯各降压药组血压进一步降低,其中以联用 ARNI 降压幅度最大,CCB 次之,β-blocker 降幅最小.RDN 可降低醛固酮(ALD)含量(P<0.001).与对照组相比,ARNI 降低 ALD 水平(P<0.001),CCB、β-blocker、DIU 对 ALD 无明显影响.RDN 可缓解心肌血管周围纤维化和心肌细胞肥大(P<0.001).结论 与单纯 RDN 相比,序贯使用 ARNI、CCB、β-blocker、DIU 可进一步降低老年小鼠血压.无论是否序贯降压药物均能改善心肌重构,减轻老年小鼠心肌血管周围纤维化和心肌细胞肥大.

      Objective To explore the effects of sequential administration of different antihypertensive drugs after renal denervation(RDN)on blood pressure,the renin-angiotensin-aldosterone system(RAAS)and myocardial fibrosis in elderly mice with angiotensin Ⅱ(AngⅡ)-induced hypertension.Methods Sixty 18-month-old male C57BL/6 mice were used to establish a hypertension model induced by Ang Ⅱ.Then,they were randomly divided into Sham group with saline(Sham-control),Sham group with sacubitril/valsartan(Sham-ARNI),Sham group with amlodipine(Sham-CCB),Sham group with bisoprolol(Sham-β-blocker),Sham group with hydrochlorothiazide(Sham-DIU),RDN group with saline(RDN-control),RDN group with sacubitril/valsartan(RDN-ARNI),RDN group with amlodipine(RDN-CCB),RDN group with bisoprolol(RDN-β-blocker),RDN group with hydrochlorothiazide(RDN-DIU)10 groups,with 6 mice in each group.Blood pressure was monitored weekly.After 4 weeks of intervention,samples were collected,including plasma,renal tissue,and myocardial tissue.Results The systolic blood pressure(P=0.003)and diastolic blood pressure(P=0.009)in the RDN-control subgroup were significantly lower than those in the Sham-control subgroup.Compared with RDN alone,the blood pressure further decreased after the sequential administration of various antihypertensive drugs.Among them,the combination with ARNI resulted in the greatest reduction,followed by CCB,and the smallest reduction with the β-blocker.RDN can reduce aldosterone(ALD)level(P<0.001).Compared with the control subgroup,ARNI couldreduce ALD level(P<0.001),while CCB,β-blocker and DIU had no significant effect on ALD.RDN could alleviate perivascular myocardial fibrosis and cardiomyocyte hypertrophy(P<0.001).Conclusion Compared with RDN alone,the sequential administration of ARNI,CCB,β-blocker,and DIU can further lower blood pressure in elderly mice.Regardless of whether sequential antihypertensive drugs are administered or not,RDN can improve myocardial remodeling,and alleviate perivascular fibrosis and cardiomyocyte hypertrophy in elderly mice.
