      目的 探讨上海市社区老年人握力和认知功能之间的关系及相关关联因素.方法 在上海市 4 个区(2 个市区、2 个郊区)中选取8 个社区,开展基于社区的老年综合评估调查研究,调查内容包括一般情况、采用多类标准化量表及工具进行的功能评估,数据收集期为2023 年6 月至2024 年3 月,共3 507 名年龄≥65 岁的社区老年人参与调查,排除不合格数据和资料不全者,最终2 902 名老年人纳入汇总分析.采用经人口学特征和健康协变量校正的多因素回归模型,系统性分析握力指标与认知功能之间的潜在关联.结果 在纳入分析的2 902 名参与者中,399 人(13.7%)表现出认知功能下降,其握力中位数为23.6 kg(四分位数间距:18.5~30.0 kg).握力<18.5 kg 与简易精神状态检查量表(MMSE)总分下降显著相关,且高龄(≥85 岁)和脑卒中后遗症是 MMSE 得分下降的独立危险因素.亚组分析显示,女性握力与MMSE 得分呈显著正相关(β=0.150,P<0.001).结论 握力降低与认知功能下降密切相关,具有作为早期认知衰退标志物的潜力.

      Objective To investigate the relationship between handgrip strength and cognitive function and its correlation factors in elderly community residents in Shanghai.Methods A community-based comprehensive geriatric assessment study was conducted in 8 communities across4 districts of Shanghai(2 urban and 2 suburban).The survey included general information,as well as functional assessment based on multiple standardized scales and tools.The data collection period was from June 2023 to March 2024,and a total of 3 507 elderly community residents aged 65 and above participated in the survey.After excluding participants with ineligible or incomplete data,2 902 elderly individuals were included in the final comprehensive analysis.A multivariate regression model adjusted for demographic characteristics and health covariates was employed to systematically analyze the potential relationship between grip strength indicators and cognitive function.Results Amongthe2902participants,399(13.7%)showed cognitive decline.The median handgrip strength was23.6 kg(interquartile range:18.5-30.0 kg).Handgrip strength<18.5 kg was significantly associated with a decrease in the total score of the mini-mental state examination(MMSE).Advanced age(≥85 years)and post-stroke sequelae were independent risk factors for lower MMSE scores.Subgroup analysis revealed a significant positive correlation between handgrip strength and MMSE scores in women(β=0.150,P<0.001).Conclusion Reduced handgrip strength is closely associated with cognitive decline and has the potential to serve as a marker for early cognitive impairment.
