| 目的 探讨新加乌梅汤联合双歧杆菌四联活菌片治疗老年溃疡性结肠炎(UC)患者的疗效.方法 2024 年1 月至12 月,纳入上海市第六人民医院南院奉贤区中心医院收治的 122 例老年 UC 患者,将受试者随机分为对照组(n=61)和研究组(n=61).对照组以双歧杆菌四联活菌片治疗,研究组在对照组的基础上联合新加乌梅汤治疗,疗程为3 个月.通过 Mayo 评分、Baron 内镜评分评估2 组疗效;比较2 组血清免疫及炎症指标、肠道菌群变化;记录2 组的不良反应发生率及 UC 复发率.结果 治疗3 个月后,与对照组相比,研究组 Mayo 评分和 Baron 内镜评分较低,抗中性粒细胞胞质抗体免疫球蛋白 G(ANCA-IgG)、白细胞介素(interleukin,IL)-33 及 IL-6 水平较低,白细胞分化抗原(CD)3+、CD4+及 IL-10 水平较高,双歧杆菌、乳酸杆菌浓度较高,肠球菌和大肠杆菌浓度较低(P<0.05).2 组治疗前后 Mayo 评分和 Baron内镜评分的差值,ANCA-IgG、IL-33、IL-6、CD3+、CD4+及 IL-10 水平的差值,双歧杆菌、乳酸杆菌、肠球菌和大肠杆菌浓度的差值差异均有统计学意义(P<0.05).研究期间,2 组不良反应发生率差异无统计学意义(P=0.713).研究组复发率较对照组低(P=0.045).结论 新加乌梅汤联合双歧杆菌四联活菌片治疗老年 UC 患者能一定程度改善患者的临床症状及黏膜愈合情况,调节肠道菌群及免疫功能,抑制炎症反应,且安全性良好. |

| Objective To investigate the efficacy of Xinjia Wumei decoction combined with Bifidobacterium quadruple viable tablets in the treatment of elderly patients with ulcerative colitis(UC).Methods From January 2024 to December 2024,122 elderly UC patients admitted to Central Hospital of Fengxian District(South Branch of Sixth People's Hospital of Shanghai City)were enrolled.They were randomly divided into control group(n=61)and study group(n=61).The control group was treated with Bifidobacterium quadruple viable tablets,while the study group was treated with Xinjia Wumei decoction in addition to the same treatment as the control group.The treatment course lasted for 3 months.The therapeutic effect was evaluated using the Mayo score and the Baron endoscopic score.Serum immune and inflammatory markers,as well as the changes in intestinal flora,were compared between the two groups.The incidence of adverse reactions and the recurrence rate of UC were also recorded.Results After 3 months of treatment,compared with the control group,the study group showed lower Mayo scores and Baron endoscopic scores;lower levels of anti-neutrophil cytoplasmic antibody immunoglobulin G(ANCA-IgG),interleukin-33(IL-33),and IL-6;higher levels of CD3+,CD4+,and IL-10;higher concentrations of Bifidobacterium and Lactobacillus;and lower concentrations of Enterococcus and Escherichia coli(all P<0.05).The differences in Mayo scores and Baron endoscopic scores before and after treatment,the differences in the levels of ANCA-IgG,IL-33,IL-6,CD3+,CD4+,and IL-10,and the differences in the concentrations of Bifidobacterium,Lactobacillus,Enterococcus,and Escherichia coli between the two groups were also statistically significant(P<0.05).During the study period,there was no statistically significant difference in the incidence of adverse reactions between the two groups(P=0.713).The recurrence rate in the study group was lower than that in the control group(P=0.045).Conclusion The combination of Xinjia Wumei decoction and Bifidobacterium quadruple viable tablets can effectively improve the clinical symptoms and mucosal healing in elderly UC patients,regulate intestinal flora and immune function,inhibit inflammatory reactions.It has good safety. |
